Participant Agreement, Release and Acknowledgement of Risk Form (Please read and be certain you understand the implications of signing)
Express Assumption of Risk associated with risks of injury inherent to lands, equipment and animals, as well as the potential for you to act in a negligent manner that may contribute to your injury of death.
I am completely aware that there is risk of injury that may be encountered during use of the facilities or during any land or water activities while visiting The Bolo Club, L.L.C. I assume all responsibility if injury occurs. I furthermore agree to abide by all rules and regulations, I agree to fully release and hold The Bolo Club, L.L.C., its agents, employees, representatives and landowners harmless from any damages or injuries, current or future.
I have and will submit for review a current negative Coggins certificate.
I will keep my dog(s) on a leash at all times, No dogs are allowed on the trails.
